• 高中 accommodations are designed to ensure success.

• The IEP team is responsible for evaluating effectiveness of accommodations and adjusting as needed. 

College

• University accommodations are designed to ensure access.
• 住宿 may not fundamentally alter the course. 
• The student is responsible for evaluating the effectiveness of accommodations and must consult with CDAR should they need to adjust accommodations. 

高中

• Parents and teachers can openly speak about the student's progress in the classroom.

• Parents can request a meeting with teachers at any time.

College

The first step to receiving services from CDAR is to complete the CDAR应用程序.  在启动应用程序之前，您需要:

1. 你的汪达尔ID和用户名. You should receive this information in your welcome packet. 
2. 您的DUO身份验证设备. The CDAR应用程序 uses multi-factor authentication to protect your information, 所以你需要你的手机或钥匙卡. 

For security, the application will time out after 30 minutes. CDAR just needs basic information to start the accommodation process, you will have the opportunity during a meeting with one of our access consultants to share more details. Once you submit your application, you will receive a confirmation email. Disability documentation must be provided by a physician, 心理学家, 听力学家, or other licensed professional appropriately qualified to diagnose your specific disability. A 504 or IEP is helpful in establishing previous accommodations, but not a substitute. 如果文件不完整, or more information is needed to determine reasonable accommodations, CDAR may request additional documentation before moving forward with the accommodation process.

1. Be on official letterhead from the professional describing the disability

3. List and explain the diagnosis/description of the medical condition, 诊断日期, 医疗状况可能持续的时间, and the functional limitations that arise from the medical condition.
